Employer

Employer Client

If you are an employer directly contracting with Self Insured Reporting, it is important to understand that we are likely to be working together using Protected Health Information (PHI) from your group medical plan.  This will require us to do a few things in order to stay compliant.

  • We need need to executed a Business Associate Agreement (BAA) between:
    • Your Company / Employer, as you are the plan sponsor of your group medical plan (The covered entity) 
    • Self Insured Reporting as a Business Associate
  • Data use release agreement:
    • This is an agreement signed by you as the Employer and Plan Sponsor which will direct us as Self Insured Reporting in regard to which of your vendors we can receive and/or provide PHI to.
    • In this document, you will list these other entities such as your Broker, TPA, Pharmacy Benefit Manager, Reinsurance General Agent, On or Near-Site Clinic, Population Health Management Company or any other relevant vendor.
Benefit Broker

Benefit Broker

If you are an Employee Benefit Broker contracting with Self Insured Reporting, it is important to understand that we are likely to be working together using Protected Health Information (PHI) from your client.  This will require us to do a few things in order to stay compliant.

  • We need need to executed a Subcontractor Business Associate Agreement (S-BAA) between:
    • Benefit Broker (Business Associate to the Employer and Plan)
    • Self Insured Reporting as your Subcontractor
  • Data use release agreement:
    • This is an agreement signed by your company as a Business Associate to the Employer and Plan and will direct us as Self Insured Reporting in regard to which of your vendors we can receive and/or provide PHI to.
    • In this document, you will list the Employer as the Plan Sponsor, their Plan and any other vendors. 
General Agent

General Agents

If you are a General Agent contracting with Self Insured Reporting, it is important to understand that we are likely to be working together using Protected Health Information (PHI) from your client.  This will require us to do a few things in order to stay compliant.
